IMPORTANT FORMS
NOTE: Sports physicals are to be completed by a physician and returned to the Athletic Office.
Please allow 7-10 school business days to process. A student's Genesis will be updated when their physical has been reviewed and approved by our school doctor.
All Medical History Forms AND Athlete Participation Forms MUST be completed prior to try outs. All Students require up to date information submitted and approved before participating.
